0419-62-001 | Approved Initial Applications for Licensure or Permitting; Renewal Applications for Licensure and Permitting

Description:  The purpose of these records is to reflect the biographical information of applicants for licensure or permitting by the Board and to permit the Board to review and where appropriate, approve completed applications for the privilege of practicing dentistry or dental hygiene in the State of Georgia or for administration of sedation in the State of Georgia. The records may include approval of applications for reinstatement of licensure or inactive licensure status. The records consist of application files, which include original applications, transcripts, self-query reports from the National Practitioner Data Bank, affidavits regarding citizenship, copies of secure and verifiable documents, license verification forms, and references.

Description:  These records document the specific nature, length, and terms of conditions of administrative discipline against a licensee. The records consist of any order issued and approved by the Board against a licensee.

0419-62-003 | Investigative and Hearing Files (Dentistry)

Description:  The purpose of these records is to document the investigation of complaints and hearings of the Board to determine any action against a licensee. Upon receipt of a complaint or upon its own motion, the Board investigates any evidence that appears to show that a licensed professional has committed an act or acts that violate the laws, rules, and regulations of the Board. The records consist of the initial complaint and may include dental records, reports from the Board’s Investigative Committee, reports from investigators, transcripts from any hearings, and a copy of any consent order or order of the Board or an Administrative Law Judge.

Description:  The purpose of these records is to reflect the legal advice provided by the Department of Law. These records include legal memoranda, briefs, and correspondence containing advice from the Department of Law.

Description:  The purpose of these records is to reflect the biographical information of applicants for licensure or permitting by the Board who either failed to meet the requirements, failed to complete a timely application, or were withdrawn. The records consist of application files, which include original applications, transcripts, self-query reports from the National Practitioner Data Bank, affidavits regarding citizenship, copies of secure and verifiable documents, license verification forms, and references. Where the applications were pending, the records may not include all of the above-listed items.

Retention:  18 months
