0419-61-001A and 0419-61-001B | Approved Initial Individual and Facility Applications and Renewals

Description:  Individual Applications: The purpose of these records is to reflect the biographical information of applicants for licensure or registration by the Board and to permit the Board to review and where appropriate, approve completed applications for the privilege of practicing pharmacy in the State of Georgia. The records may include approval of applications for reinstatement of licensure or inactive licensure status. The records consist of application files, which include original applications, transcripts, NAPLEX results, internship reports, affidavits regarding citizenship, affidavits of application information, copies of secure and verifiable documents, license verification forms, and consent forms for background checks. Facility Applications: The purpose of these records is to reflect basic information of facility applicants for licensure, permitting, or registration by the Board and to permit the Board to review and where appropriate, approve completed applications for the privilege of practicing pharmacy in the State of Georgia. The records may include approval of applications for reinstatement of licensure, permitting, or registration. The records consist of application files, which include original applications, affidavits regarding citizenship, affidavits of application information, copies of secure and verifiable documents, license verification forms, names of owners, officers, and pharmacists-in-charge, and personnel certification forms.

Retention:  (0419-61-001A) Individual Applications and Renewals: 50 years; (0419-61-001B) Facility Applications and Renewals: 10 years after facility closes.

Description:  These records document the specific nature, length, and terms of conditions of administrative discipline against a licensee, registrant, or permit-holder. The records consist of any order issued and approved by the Board against a licensee, registrant, or permit-holder.

0419-61-002A and 0419-61-002B | Examination Grade Sheets and Materials for Applicants for Pharmacist Licensure

Description:  The purpose of these records is to reflect applicants' for licensure completion of practical examinations for licensure. The records consist of score sheets and paper examination materials, such as question prompts and the results of evaluations by the Board and its agents.

Retention:  (0419-61-002A) Applicants for Pharmacist Licensure: Approved: 10 years; (0419-61-002B) Failed Examination Grade Sheets and Materials: 18 months

0419-61-005 | Investigative and Hearing Files (Pharmacy)

Description:  The purpose of these records is to document the investigation of complaints and hearings of the Board to determine any action against a licensee, registrant, or permit-holder. Upon receipt of a complaint or upon its own motion, the Board investigates any evidence that appears to show that a licensee, registrant, or permit-holder has committed an act or acts that violate the laws, rules, and regulations of the Board. The records consist of the initial complaint and may include pharmacy records, reports from the Board’s Cognizant Member, reports from agents of the Georgia Drugs and Narcotics Agency, transcripts from any hearings, and a copy of any consent order or order of the Board or an Administrative Law Judge.

Retention:  50 years

0419-61-004 | Legal Opinions from the Department of Law (Pharmacy)

Description:  The purpose of these records is to reflect the legal advice provided by the Department of Law. These records include legal memoranda, briefs, and correspondence containing advice from the Department of Law.

Retention:  Retain for useful life.

Description:  The purpose of these records is to reflect the biographical information of applicants for licensure, registration, or permitting by the Board who either failed to meet the requirements, failed to complete a timely application, or were withdrawn. The records consist of application files, which include original applications, affidavits regarding citizenship, affidavits of application information, copies of secure and verifiable documents, license verification forms, names of owners, officers, and pharmacists-in-charge, personnel certification forms, transcripts, NAPLEX results, internship reports, and consent forms for background checks. Where the applications were pending, the records may not include all of the above-listed items.

Retention:  18 months
